ACDA-MN-Short-NoText-181x75-RichBlkCongratulations on your child being selected for one of the most prestigious honors in choral music, that of becoming a member of the ACDA-MN State Honor Choir program!

There are two important items for you to accomplish within the next several weeks.

1 — Secure the services of a Notary Public to witness your signing of the Student Registration Form

Student Registration Form

The specific requirement for parents/guardians to secure a notarized signature on the honor choir registration form, ensures that all such signatures are not forged, but are valid, and that the signatories are in agreement with all the conditions and policies presented on the form. It is the only way for ACDA-MN to verify that the signatory is that of the parent/guardian. Notary Publics are located in local banks, government offices, school or district offices, accounting firms, and most UPS stores. There are no fees related to securing a signature of a Notary Public.

2 — Submit payment of $124 by check payable to your child’s institution/school

Return payment and notarized form prior to the posted deadlines to your child’s choral director:

4-5-6 Honor Choir Wednesday, March 28

The student registration fee will cover the following expenses: Guest conductor artistic fees and travel expenses, accompanist, instrumentalists, rental of performance and rehearsal facilities, liability insurance, food, T-shirt, CD recording of the concert, two tickets to the concert performance, and administrative support services.

Parents please assist our state honor choir program by monitoring your student’s regular progress in rehearsing and thoroughly preparing the music.

Please see the Rehearsal Track section for the required music.

  • Scholarships are available for Registration Fees. Please see “SCHOLARSHIPS AVAILABLE” section for details.
